WE7.4 A Systematic Review of Splenic Viability Following Arterial Embolisation in Trauma

Abstract The spleen is the most commonly injured organ in blunt abdominal trauma. Patients with these injuries are increasingly being managed nonoperatively, use of Splenic Artery Embolisation (SAE). Many patients receive lifelong prophylactic antibiotics as though they were asplenic. aim to seek robust evidence splenic vascular integrity and immunocompetence following SAE, consider whether antibiotic prophylaxis necessary for trauma this way. 2 separate literature reviews carried out using Medline database. One assess pertaining tissue viability embolisation, other find reviewing immune function after embolisation. In arm, 21 articles reviewed. 12 studies observed no infarcts at all. 18 noted <10% their populations. Distal embolisation resulted more infarcted areas than proximal. 9 performed follow-up imaging confirmed post-procedure. For review immunocompetence, search 13 full text articles. There was consistency immunologic marker used test post-SAE, but 11 preserved by respective markers. data do suggest immunocompetence. Agreement yet be reached which definitively assesses length needed investigate it. More research required confirm suggestion from that unnecessary treated SAE.
